带状疱疹神经痛的症状有哪些

带状疱疹后遗神经痛(PHN)就是急性带状疱疹(AHZ)后遗症。即急性带状疱疹临床症状治愈后延续疼痛超过一个月的神经疼痛病。它的病理改变已不同于带状疱疹，是一种独立的疾病。

1、剧烈神经痛是带状疱疹的主要特征，是由于带状疱疹病毒的亲神经性，侵袭感觉神经末梢造成的，可在发疹前或伴随皮疹出现，少儿不明显，青年人略轻，老年人较重。

2、当皮损症状完全消退后， 会有约40%左右的患者遗留有神经痛，而60岁以上老人是后遗神经痛的高发人群，75岁以上高寿老人几乎都会留下后遗症。

3、疼痛以胸段肋间神经和面部三叉神经分布区域多见，四肢疼痛会沿神经游走。

发病机理：根据发病临床表现与病理学研究，专家们分析发病机理大概有以下几方面：

1、带状疱疹发病引起了周围神经病变及带状疱疹性神经痛;

2、因带状疱疹并发症之一是无菌性脑膜脑炎，说明带状疱疹会累及中枢神经系统，部分患者临床采用阻滞疗法后，仍有疼痛表现就是证明;

3、带状疱疹并发的精神症状，在皮损消退后仍伴随着神经痛(失眠、精神沮丧、精神消沉)。也就是说，神经疼痛既有生理原因，又有心理因素。但主要原因是：患者没有足够的营养来提高免疫力，使被病毒破坏的神经细胞没有得到及时有效修复，被破坏的组织和细胞形成的垃圾无法及时清理排出，病毒进一步繁殖破坏神经组织。

发病人群：据报道，带状疱疹发病率为人群的1.4‰-2.8‰之间，约有40%左右的患者遗留有神经痛。60岁以下患者约占受累人数的25%左右，60岁以上老年人是带状疱疹后遗神经痛的主要人群，约占受累人数的75%左右，75岁以上老人几乎都有后遗症，且治疗难度大。该病是医学界的治疗难题，是中老年人健康潜在的杀手。随着保健知识的普及和经济条件的改善，大多数患者不再被动忍受，而是主动求医治疗。

主要特点：带状疱疹后遗神经痛的主要特点就是剧烈的、顽固性的疼痛。带状疱疹皮损消除后疼痛仍持续，轻微的刺激即引起疼痛发作，不刺激也会突然发作。

1、为减轻衣服对身体的刺激，有人不敢穿衣，或把衣服撑起来;

2、晚上间歇性疼痛，整夜的睡不好觉。

3、如病毒侵犯到相应脑神经会影响视力、引起面瘫和听觉障碍。

4、除疼痛外，还会诱发心脏病、脑出血、甚至导致死亡。

5、这种神经痛持续的时间较长，病程短则几个月，长者5-6年，国内权威报道为9年。牛德兴主任曾接诊过一例疼痛长达19年的患者，两例16年的患者，还有两位疼痛难忍，跳楼造成瘫痪的患者。
